Dutch government wants every citizen to register at the Donorregister to declare his or her intentions regarding organ and/or tissue donation after death. This occurs through a Donor form.

Mijksenaar was commissioned to redesign the exisiting form. A new form has to be clear/conspicuous and more attractive then the more more functional exisiting form. Additionally, a requirement for the form is that it can be filled out easily and faultless.
